Why We Built This

DOT physicals aren't complicated in concept — but the regulations are buried in 49 CFR 391.41, written in federal legalese, and spread across multiple FMCSA advisory documents. Most drivers walk into their exam not knowing whether their blood pressure reading will cost them a 2-year card or a 90-day certificate. That's a significant difference when you're running your own route or managing a fleet's renewal schedule.

We built DOTs Calculator to translate those regulations into plain language and give drivers a free, accurate pre-exam screening tool. You enter your numbers — blood pressure, weight, height, vision, hearing, diabetes status — and we tell you exactly where you stand against the FMCSA thresholds in 49 CFR 391.41(b).

The tool doesn't replace a licensed medical examiner. Only a National Registry of Certified Medical Examiners (NRCME) examiner can issue a Medical Examiner's Certificate. But it does let you walk in prepared — knowing what questions will come up and which numbers matter most.

How the Calculator Works

Every calculation in the DOT Physical Calculator is derived directly from federal regulations and the FMCSA Medical Examiner Handbook. Blood pressure thresholds follow the four-tier certification system: normal (under 140/90) earns a 2-year certificate, Stage 1 hypertension (140–159 / 90–99) typically results in a 1-year certificate, Stage 2 (160–179 / 100–109) triggers a 90-day certificate, and Stage 3 (180+ / 110+) results in disqualification until treated.

BMI is calculated using the standard formula (weight × 703 ÷ height²) and flagged at the FMCSA's sleep apnea screening threshold. Vision results are mapped to the 20/40 corrected-acuity requirement per CFR 391.41(b)(10). Hearing is assessed against the forced-whisper standard at 5 feet. Diabetes status is evaluated using the FMCSA's insulin-treated diabetes mellitus (ITDM) exemption framework.

We update the calculator whenever FMCSA guidance changes. The current version reflects the Medical Examiner Handbook (2022 edition) and applicable Federal Register notices through Q1 2026.
